Students and affiliates who were treated or transported by Columbia University Emergency Medical Service (CUEMS) and need a copy of their health records from Columbia Health Medical Services must submit an Authorization for the Release of Health Information form. This form allows Columbia Health to securely release your medical information to you or to a designated third party. Once the completed form is submitted, requests are typically processed within approximately 10 business days.
